• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Navigate your internet browser to https://www.zendesk.com/contact/
2. Fill out the page with your information where it requested to do so
3. Request cancellation in message box
4. Click on contact sales

Follow these steps:

1. Contact the business at 888-670-4887
2. Advise the representative that you want to cancel your account
3. Ask for a verbal confirmation code or email as proof of your cancellation
4. Ask the representative if you will be charged again
5. Retain the confirmation information you’re given
